Medicaid vision screening process opens for adults as well as children

Pago Pago, AMERICAN SAMOA — The Medicaid office will open up its screening process for all eligible adult and child beneficiaries, for the New Vision program beginning Monday, October 3, 2022.

 Screening will be available at the ASTCA Executive Building, 3rd-floor, Suite #306, from 8 am to 3 pm Monday thru Friday, and after hours from 4 pm to 6 pm on Tuesdays and Thursdays only.

“We encourage the public to please provide all required documentation to avoid delays in processing your enrollment,” the Medicaid office detailed in a press release issued this week.

A list of documents and information you must bring with you at the time of enrollment include:

Children 21 and under:

1. Immunization records

2. Birth certificate

3. AS Immigration ID (for foreign children)

Adults (U.S. National/ U.S. Citizen/ U.S. Permanent Residents)

1. Two forms of identification — Passport & Driver's License or American Samoa ID (Vitals)

2. Birth certificate

The Medicaid Office also requires a working phone number and email address on file for communication purposes.
